Contest Rules

It is the entrant’s responsibility to make sure all rules and eligibility requirements are met.

  • Videos can be funny or serious, as long as they focus on the contest message for each category.
  • Videos must be accompanied by an entry form (link to entry form). This form must provide valid contact information.
  • Videos must be no more than 3 minutes (180 seconds) long.
  • Entries must be received by 11:59 p.m. EST on November 20, 2011.
  • You must be at least 13 years old to enter.
  • SeventyK employees, contractors, or grantees and their immediate family members may submit videos, but are not eligible to win. Immediate family members include spouses, siblings, parents, children, and any other people residing at the same household location, whether or not they are related.
  • Entrants have agreed to these terms and conditions, or a legal guardian must agree to these terms and conditions if the entrant is a minor (under age 18).
  • The contest is subject to all applicable federal, state and local laws and regulations.
  • Videos must not contain offensive material, violence, profanity, sex, or direct attacks on individuals or organizations.
  • Your video may be produced with assistance (financial or otherwise) from a second party but you must disclose this on the contest entry form.
  • The video must be your own original creation. No copyrighted music, video, or images may be used in submissions to this contest.
  • Videos that are not recorded in English must include English-language captions. Videos submitted in sign language must also contain English captions.
  • Everyone appearing in the video must have given his/her consent.
  • Videos must not infringe on any third party rights.
  • Videos must not have been produced for compensation or posted on any other Web page.
  • By submitting a video to this contest, you grant to SeventyK a royalty-free license to copy, distribute, modify, display, and perform publicly and otherwise use, and authorize others to use, your video for any educational purpose throughout the world and in any media.
  • Prior to public voting, if your video or image follows the rules and meets the eligibility criteria above, it will be judged by a panel based on creativity and originality, content of message, relation to this year’s theme, and video/audio quality.
  • The decision of the judges is final.
  • The top ten videos for each of the two categories will become finalists. All finalists will be notified by email on November 25, 2011.
  • The top 20 finalist videos will display on the Stop A Doc YouTube Channel.
  • The voting period begins at 12:00 noon EST on November 25, 2011.
  • Only one vote per person. Non-human votes will not count.
  • Only votes that are received in the form of LIKES that are cast between 12:00 noon EST November 25, 2011 through 11:59 p.m. EST on December 23, 2011 will count.
  • Winners will be notified by e-mail and announced on the contest Web site on December 30, 2011.
  • Top 10 finalists must provide a copy of the original video file to SeventyK to receive an award.
  • SeventyK reserves the right to not select a winner if none of the entries received are judged to meet Rules and Eligibility criteria.
  • SeventyK reserves the right to disqualify any video that it deems inappropriate for any reason and/or does not meet contest requirements.

Timeline

  • Deadline for submissions – 11:59 p.m. EST, November 20, 2011
  • Announcement of top 10 finalists for each category – November 25, 2011
  • Public voting period – 12 noon EST, November 25 until 11:59 p.m. EST December 23, 2011
  • Winners announced – December 30, 2011

  • Patients, caregivers or interested individuals ask their healthcare providers the five important questions about adolescent and young adult (AYA) cancer.
  • Healthcare professionals or students demonstrate that they know the five points regarding the unique needs of adolescent and young adults (AYAs) with cancer.
